[Artemisia] Minor Forms required to attend Estrella War

Recently, there have been many contradictory postings on lists about what forms are required for minors attending events in Atenveldt. This posting is intended to clarify that issue.

These forms are used for minors attending events in Atenveldt:
- SCA MINOR WAIVER or FAMILY MINOR WAIVER
- TEMPORARY GUARDIAN FORM
- MEDICAL AUTHORIZATION FOR MINORS

MINORS ATTENDING WITH PARENTS:
A minor attending an SCA event with their parent or legal guardian must have
an SCA MINOR WAIVER filed with the Troll Booth when they enter the event, OR
the minor must be included on a FAMILY MINOR WAIVER filed with the Troll
Booth when the family enters the event. This form does not need to be
notarized.

MINORS ATTENDING WITHOUT PARENTS:
Minors may attend SCA events with a temporary guardian appointed by their
parent/legal guardian for the event if they provide two (2) copies of the
three (3) forms listed below at the Troll Booth when they enter the event.
One copy of these forms is retained by the SCA at the Troll Booth, and the
temporary guardian/minor is asked to retain the other copy during the event.
The three required forms are:

1. An SCA MINOR WAIVER for the minor signed by the parent/legal guardian.
2. A notarized TEMPORARY GUARDIAN FORM (from the jurisdiction where the parent/minor lives) signed by the
parent/legal guardian.
3. A notarized MEDICAL AUTHORIZATION FOR MINORS FORM for the minor signed
by the parent/legal guardian.

Please note that the forms in item 2 and 3 above MUST BE NOTARIZED before
you come to the event to be accepted.
