Apologies if anyone's already mentioned this, but the book in question wasn't commercially published. The printer was the notorious scam / vanity operation PublishAmerica. So the text underwent none of the editing and review that a real publisher would have provided - or even the screening of going through the slush pile. 
PublishAmerica also provides no marketing, and usually their books sink without trace, unless a desperate author can create her own publicity.
