Die älteste bekannte Abbildung von Holzschuhen ist auf einem Altarretabel aus dem 15. Jh. von Derick Baegert in der Dortmunder Propsteikirche zu sehen.

(according to sources out of Germany, the earliest representation of wooden shoes is on the Altarpiece in the Dortmund Propsteikirche.
It was created in the 15th Century by Derick Baegert.  It is shown in the following link:
